Falling can significantly impact confidence in engaging in meaningful daily tasks and even cause hesitance or avoidance altogether of activities due to fear. Here at Generations Home Health, our practitioners strive to help empower all of our patients to maximize their quality of life by taking an active role in their own fall prevention.

According to the Centers for Disease Control and Prevention (CDC):

  • 1 out of 5 falls cause SERIOUS injury
  • 800,000 patients are hospitalized each year because of a fall
  • 300,000 patients are hospitalized for hip fracture as a result of a fall
  • Falls are the most common cause of Traumatic Brain Injury

  • Urinary Tract Infections/Incontinence

What you can do to lower risk of falling:

  • Follow medicine recommendations from your doctor
  • Follow recommendations from your home health therapy team regarding home safety, equipment and home exercise program
  • Ensure that lighting fixtures are in working order and lamp switches easily accessible
  • Visit your eye doctor annually and follow their recommendations regarding visual aids
  • Continue to assess your home environment for potential hazards
  • Wear shoes that are well fitting: supportive and secure with laces/velcro closures, lightweight, low heel, and have a thin/textured/ slip resistance sole
  • Have your doctor check your feet every visit

Falls Free Checkup Tool

This falls questionnaire is a validated 12 question tool that will help determine if you or the person you are caring for is at risk for falls and if further action is needed to prevent them in the future.

An individual score of greater than 4 indicates that you or the person you care caring for may be at risk for a serious fall.

Upon completion of this screening, you have the option of entering your email to obtain a personalized report that will be available to you in as little as 3 minutes. The email will include a letter with your score results that you can take with you to your provider, resources to take action to reduce your fall risk, resources to link you to community resources, and a conversation guide for caregivers.
